Step 1:
 Fill in the form to the right and click "Continue to Step 2" at the bottom

Step 2
 Once you are redirected to a second page, select the appropriate class for verification and payment through Google Checkout SSL.

Thats it! 
You will receive a confirmation email after completing payment.
